"Patient-Centered Medical Home: From Theory to Practice"
Lecture Synopsis:
The Patient-Centered Medical Home (PCMH) is a model of care that emphasizes care coordination and communication to transform primary care into “what patients want it to be.” National Committee for QualityAssurance (NCQA) PCMH Recognition is the most widely adopted model for transforming primary care practices into medical homes. Research confirms medical homes can lead to higher quality and lower costs,and can improve patients’ and providers’ experience of care.

Guest Speaker, Michael S. Barr, MD, MBA, MACP is a board-certified internist and executive vice president for the Quality Measurement & Research Group at NCQA. His portfolio at NCQA includes performance measurement development; research; managing NCQA’s contracts and grants portfolio; and contributing to strategic initiatives, public policy and educational programs. Prior to joining NCQA in 2014, Barr was senior vice president, Division of Medical Practice for the American College of Physicians, where he was responsible for promoting patient-centered care through development of programs, services and quality improvement initiatives for internists and other health care professionals.
From 1999–2005, Barr was chief medical officer for Baltimore Medical System, Inc., a Joint Commission accredited Federally-Qualified Health Center. He practiced internal medicine full time in the Division of General Internal Medicine at Vanderbilt University from 1993–1998 and held various administrative positions, including physician director, Medical Management Programs, for the Vanderbilt Medical Group. From 1989–1993, Barr was an active duty physician in the United States Air Force at Moody Air Force Base, Georgia.
Barr has a BS in forest biology from the State University of New York, College of Environmental Science and Forestry. He attended New York University School of Medicine through the U.S. Air Force Health Professions Scholarship Program, completed his residency in internal medicine at Rush-Presbyterian-St. Luke’s Medical Center in Chicago and earned an MBA from the Vanderbilt Owen Graduate School of Management.
Barr was a commissioner on the Maryland Health Care Commission (2013–2015), previously served on the Health Information Technology Policy Committee Meaningful Use Workgroup (2010–2012) and is currently on the Board of Trustees of The Horizon Foundation of Howard County.
Co-Speaker, Jasmine Chao, DO, MS, FAAFP, FAAIM is an experienced and energetic physician, who has rich clinical experience with in-patient, outpatient and sub-acute rehab settings. She provides comprehensive medical care to patients including those who have had little or no prior medical care. Dr. Chao has full admitting privileges to Evanston, Glenbrook, Highland Park, Skokie, St Francis (in Evanston) and Advocate Lutheran General Hospitals. She prides herself in quickly and correctly assessing critical health problems, then gathering and coordinating the appropriate specialists, to seek the best outcomes for her patients. A consummate team player, Dr. Chao is skilled at managing difficult personalities and encouraging cooperation and compliance with medical treatment plans.
Coming to medicine from a basic sciences research background in molecular biology and immunology, Dr. Chao is uniquely suited to provide expert supervision of clinical studies involving patients of all ages and pathologies. She earned a Masters of Science from the University of Illinois, Urbana, prior to leaving the Ph.D. program to pursue her interest in medicine and earned her D.O. degree from the Chicago College of Osteopathic Medicine, and completed her residency at Northwestern University in Family Medicine.
Dr. Chao practices in the North suburb of the Chicago Metropolitan area and her practice has earned the National Committeee for Quality Assurance (NCQA) certification in 2016. Since 2008, she has been a diplomat in Geriatric Medicine with ABPS and is currently serving as the President of the AAPS Foundation.
